Tricare (please provide a copy of authorization, don't assume we have it)
USFHP
Blue Cross Blue Sheild 
Premera
Regence
Aetna
Cigna
Medicaid 
Kaiser PPO plans

Life Span Occupational Therapy currently does not accept the following:
​Medicare plans (we are unable to work with any primary or secondary plans) or United Healthcare plans

If you wish to change your Medicaid plan you can visit:

Make sure you regularly check your status and renew your coverage as needed.

Please email Life Span for intake paperwork and insurance verification forms or get in touch with us via our contact page, please be aware that some insurance plans require pre-authorization  and a referral before you can start therapy.
