Life Span Occupational Therapy accepts most insurance plans and has very affordable private pay rates. If you do not see your plan here, please contact us for more information, we bill insurance as a curtesy to you, please make sure you know your plan and if your treatment will be covered:

• Tricare/ ECHO
• US Family Health Plan
• Cigna
• Premera
• Regence
• BCBS
• Aetna
• First Choice
• L&I WA ( depending on management group please check for more details)
• PIP
• Apple Health Medicaid plans (Coordinated care, Molina,  Amerihealth, Community Health Plan of WA, Provider one ) 
• Plans not shown may be able to be billed out of network or with special authorization. Ask for more details. 
• Specific grants for therapy i.e.: United Health Children's fund.
• Private pay options available

Life Span Occupational Therapy currently does not accept the following:
​Medicare plans (we are unable to work with any primary or secondary plans) or United Healthcare 

If you wish to change your Medicaid plan you can visit:

Make sure you regularly check your legibility and renew your coverage as needed.
